Identification numbers:12139907     I resided in apt.  105,  3230 E. Roosevelt street,  Phoenix, AZ. 85008 from September, 1996 - September, 1997. As an inpatient in E.C.M.C. , 4th floor psychiatry, 462 Grider Street, Buffalo, N.Y. 14215, since my mother called Buffalo police for no reason other than she wanted me hospitalized, I mailed monthly rent checks drawn on a Norwest bank checking account ( I ordered checks to be mailed to the hospital ) to management of Celtic Property Management. I also paid the Arizona Public Service electricity bills from hospital, while someone from Celtic Property Management did the favor of flipping a switch to disconnect electric service to apt. 105. I was discharged from E.C.M.C. about April, 1998, to 336 Forest Ave., 14213, Emergency Hostel Service ( Transitional Services , Inc. ), where rent was paid in advance and boarded a Greyhound bus to return to apt. 105, heavily medicated from unwanted Fluphenazine I.M. injections. I felt sick and boarded a bus to Buffalo same day of arrival in city of Phoenix. There probably was a refund owed to me by A.P.S. and maybe an A.P.S. check was sent to me at some city of Buffalo address during 1998 for tenancy in apt. 105, I am unsure. There was a $90,000 hospital bill unpaid and discharged in 2008 Bankruptcy Court, Buffalo, N.Y.  Attorney George Navaugh, who lived in East Aurora, N.Y. , I believe, died March 17, 2009 or March 17, 2010. His obituary was read by myself holding a copy of The Buffalo News inside the Strozzi Building, a client of The Buffalo Psychiatric Center, 400 Forest Ave., Buffalo, N.Y. 14213. George Navaugh, who  represented me, Robert T. O'Brien, in that Bankruptcy, did not charge a fee.  Upon returning to Phoenix, June 2010- August 8, 2011, I resided in apt. 155, 3230 E. Roosevelt Street, Phoenix, AZ. 85008-5117. I overpaid never reimbursed rent 1 1/2 months at rate of $325.38 per month,  and wasn't able to cash an A.P.S. security deposit refund check, an adjusted amount, originally $250.00 paid with a bank card by Kathleen A. Platek, rep. payee for Robert T. O'Brien, in amount of $191.86.  Kathleen Platek, d.o.b. 03/13/1967,  was then living at 250 Hilltop Rd., Elma, N.Y. 14059-9412 with her husband Robert Platek and daughters Ashley Platek & Courtney Platek. Kathleen Anne Platek later moved to 94 Queens Drive, Grand Island, N.Y. 14072 after a break up or divorce from her husband, Robert Platek.   A Chase bank Arizona Public Service electricity utility company refund check payable to the only tenant, myself, the author typing this text, with never even one visitor outside of managements' employees to remove & replace air filters or Ken McKinney to remove my property with his friend, a pickup truck driver, or one visit/knock on door 155 from manager Ms. Renee Heinz seen once also walking with her boss, a man, in the 'C section apartments far left in the back pat the other parking lot and garbage dumpster green 'after my telephone messages to (602)914-1299  ( tenant of the studio or no bedroom, no mattress,  apt. 155, Robert Thomas O'Brien d.o.b. 06/21/1964 5 29 HOURS city of Buffalo account numbers-final five****96284, 3230 E. Roosevelt Street, Phoenix, AZ. 85008-5117, June 07, 2010 although arrived via Greyhound Bus 06/02/2010 resided temporarily apt. 'C', with occupant Mr. Ken A. McKinney until 06/07/2010, in & out before he became upset, an emoloyee of suite16, 5555 N. 7 th Street, Phoenix, AZ. 85014 called KARSH'S BAKERY" where he received mail including a large box of clothes and also received mail at his post office box on E. McDowell Rd., including return receipt requested green cards that he signed returned to me in Buffalo, N.Y. for 9 cartons of cigarette shipments, many shipments, over many, many years-he never paid me for..judgeLarkin ruled his favor? on his landlady's 2721 E. Willetta Street, Phoenix, AZ. 85008 who knew Mr.Collin Kenney, of same street address , alone inside apt.'A'-August 08, 2011, two six month leases satisfied, December  not leased, last lease expired May 31, 2011  *however , rent was prepaid through September 30, 2011 at rate of $325.38 per month,  mngr. Ms. Renee Heinz, representing landlord doing business as " Coral Gardens Apts./Celtic Property Management " never refunded * to myself nor Kathleen Platek d.o.b. 03/13/1967  prepaid rent by me ),  electricity utility company account check number 0736267, in amount of $191.86, pay to the order of Robert O'Brien, is nonrefundable, the refund check expired as I interpret the loss.  Finance Technician, Tanya Brito, with Arizona Public Service, in a letter she signed to me, addressed to: 6525 Rosemead Blvd., apt. 3, bed 'A'; Pico Rivera, CA. 90660-3501, said in other words, this case is closed, if  I wanted to then I may contact Fraud/Forgery Department Arizona Public Service ( electric utility company ) at telephone 18665642262.
  Elsewhere on the web-- Town of Tonawanda, N.Y. 14150 near Niagara Falls Blvd. there was/is a Volunteer Fire house on Dexter Terrace Rd.  As told by Irv Weinstein channel 7 t.v. many years ago, that Dexter Terrace Rd. Volunteer Fire Company House would sound off a siren daily or seven days a week at 1700 hours. The siren was loud and would continue its noise for several minutes. There was never a fire at 1700 hours requiring that Fire Company's volunteers to respond to and put out. It was a tribute or remembrance regarding / noting the victims of WW11  who died in Germany's Nazi Concentration camps. The noise generated by the siren affected my ability to think clearly. I should have used ear protection. Our family at 53 Leawood Drive was a distance from the source of the noise.
               James & Ruth O'Brien; 53 Leawood Drive; Tonawanda, N.Y. 14150, were Marine Midland bank customers. Their previous address was a Duluth Ave. Kenmore , N.Y. 14217 house. Myself, Robert T. O'Brien, was born 06/21/1964 Buffalo, N.Y. 14208. My sister, Katy ( O'Brien ) Tober, was born 02/09/1966. Soon after Katy was born , our family of four moved from the town of Kenmore to the Town of Tonawanda. Dad said we could not afford to live there. However, mom wanted to live there.  My mother told me that furniture had been moved inside our house at 53 Leawood Drive while we were away in the Adirondack mountains/ cabin rentals. However, nothing was removed from the house. Uninvited guests entered through an unlocked side door(s). The intruder(s) were gone when we returned from the Adirondack mountains.  I remember feeding the chipmunks near trees while in the Adirondack mountains. Sister Kathleen wasn't born until March 13, 1967, brother Jim born 02/12/1969, & brother Kevin was born 12/14/1970.
     I wish to learn when I was there in the Adirondacks on vacation and where I stayed and for how many days, sometime/someplace/duration estimating between 05/05/1966-12/13/1966??
                     I'll describe details past and present of my representative payee status. I'm not my own representative payee. In other words, the two S.S.D. checks are not paid to the order of Robert T. O'Brien. Instead, the S.S.D. checks are paid to a representative payee, currently Kathleen Platek, d.o.b. 03/13/1967, formerly of , with ex husband, Robert Platek,  & daughters, Ashley Platek, and Courtney Platek , 250 Hilltop Rd., Elma, N.Y. 14059-9412 . Now,  Mrs. Kathleen Platek resides 94 Queens Drive, Grand Island, N.Y. 14072; telephone mobile Mrs. Kathleen Platek 716-812-2089 mobile. Kathleen Platek accepted the responsibility of paying Robert T. O'Brien's , d.o.b. 06/21/1964, bills . Robert T. O'Brien transferred from himself , as the original policyholder, of policy  41109646 policy number, as purchased from Gerber Life Insurance Co. , originally 1/19/2009 first payment made to Gerber Life using a telephone. He appointed Mrs.  Kathleen Platek,  d.o.b. 3/13/1967,  replacing himself, d.o.b. 06/21/1964,  as policyholder. Mrs. Kathleen  Anne Platek, d.o.b. 3/13/1967,  remains the policyholder, today's date Saturday, 9/30/2017. The transfer of policyholder status was made using Fax machines, one she controlled, 8557891395 near Buffalo, N.Y.  She immediately, same day , I was told, upon my encouragement, signed as policyholder and primary beneficiary, then faxed the changed policyholder form that I signed having received via mail delivery or fax, from Gerber Life fax 2139283078, then faxed to Kathleen A. Platek from Meadowbrook Manor. Her name as the new policyholder as well as primary beneficiary.  One fax machine a nurses' aid or other employee accessed, unnamed here, who worked near Mr. Ryan's office, agreed to use  to fax the form to Kathleen A. Platek. One of them faxed for me, the form, that I was allowed to verify as received at fax number 8557891395 by Kathleen A. Platek. I was grateful to have accessed the fax machine as an inpatient inside Meadowbrook Manor Behavioral Health Center , a mental health rehabilitation facility, 3951 East Blvd., Los Angeles, CA. 90066.  I was an inpatient there before 9/25/2014, my arrival date into 6525 Rosemead Blvd., Pico Rivera, CA. 90660-3501 . I was transferred to Meadowbrook Manor from Kedren Mental Health facility as an inpatient there Kedren patient file number: 118087, patient Robert O'Brien d.o.b. 06/21/1064, discharged 1/15/2014, Mark Pushin M.D. signing transfer papers to Meadowbrook Manor facility, 3951 East Blvd., Los Angeles, CA. 90066-4605. She has agreed to produce and copy and send return receipt requested certified mail, handled by herself, in person, at a U.S. post Office, three items.1.) Seek his death certificate -paying the fee. 2.) acquire an obituary newspaper clipping or death listing newspaper clipping, and 3.) submit to the proper claims agent representing Gerber Life Insurance Company also, #3,  a copy of the policy 41109646, she holds as policyholder, the "insured Robert Thomas O'Brien d.o.b. 06/21/1964."  Claims are handled by a  N.Y. state Gerber Life Insurance Company business address , that handles claims arising from the policy issuer, the same company, Gerber Life Insurance, at a  ___?445 State Street, Fremont, Michigan 49412 address . Claims should be sent using a U.S. carrier via a certified, return receipt requested United States Postal Service issued card that attaches to mail piece glued. An original copy of the Gerber Life Policy -41109646 policy number, shall be sent to the proper address of the claims handler of Gerber Life insurance Company, 1-800-253-3074,  which she now holds in a known storage area, a ten year term policy started 01/19/2009, by Robert Thomas O'Brien who made the first payment with his bank debit card from a telephone inside the Strozzi Building, B.P.C. 14213. James J. O'Brien, a visitor, delivered his ( brother, inpatient Robert T. O'Brien patient number 086456 ) bank cards to Robert during a visit covertly. I believe that I mailed or otherwise transferred/permission to handle my wallet with my bank cards to James J. O'Brien prior to the October 25, 2008, hospitalization into Buffalo General Hospital-moved to B.P.C., Strozzi Bldg., 400 Forest Ave., Buffalo, N.Y. 14213. Money orders signed by Robert O'Brien then mailed, or prepaid debit cards or previous credit card payments by telephone mostly were made by Robert O'Brien, the insured, to Gerber Life agents. The premiums currently paid $83.55 monthly rate, subject to change, probably increase with age after ten year term ends despite being a newly declared nonsmoker 10/15/2016 premiums paid through 08/19/2019 by "the insured Robert Thomas O'Brien", policy holder currently Kathleen Platek, as well as primary beneficiary. Contingent beneficiary Katy Tober. Mrs.  Kathleen Platek must claim life insurance benefits on his insured life, insurer Gerber Life policy number 41109646.

Platek, d.o.b. 3/13/1967.  Kathleen Anne Platek became my rep. payee for Robert T. O'Brien when Kevin Patrick O'Brien, d.o.b. 12-14-1970, formerly of 586 Emerson Drive, Amherst, N.Y. 14226, with his wife Krysten O'Brien moved out of New York state to the state of Delaware, 22514 Lakeshore Drive, Georgetown, Delaware 19947, mobile telephone 7169138855. Krysten Tober previously resided before that of a Robin Hill Rd. address , Williamsville, N.Y.   14221. Kevin's father-in-law Francis Tober with his son, Dave Tober, and another sister to Dave & Krysten Tober, lived in the Robin Hill Rd. address of Williamsville, N.Y. 14221. Dave Tober who married my sister, previously Katy O'Brien, d.o.b. February 9, 1966, moved to 22497 Lakeshore Drive, Georgetown, Delaware 19947. They lived with their two sons and two daughters, Spencer Tober, Jim Tober, Allison Tober, & Madison Tober.
